Study of Interaction between Sleep-disordered Breathing and Acute Ischemic Stroke

Abstract
Sleep-disordered breathing (SDB) is suggested to have a strong association with ischemic strokes. Risk factors, stroke subtypes, stroke lesion distribution, and the outcome of SDB in stroke patients are not fully studied in Korea. We prospectively studied 293 patients with acute ischemic stroke. Cardiovascular risk factors, stroke severity, sleep-related stroke onset, distribution of stroke lesions, and 3-month modified Rankin scores (mRS) were assessed. Stroke severity was assessed by National Institute of Health Stroke Scale (NIHSS) and mRS. The apnea-hypopnea index(AHI) was determined 6.3 ÷Ï 2.2 days after stroke onset with a portable sleep apnea monitoring device. The incidence of SDB was 63.1%. The SDB group showed older age, higher NIHSS and mRS, more bulbar weakness, and higher incidence of sleep-related stroke onset. Among risk factors, alcohol consumption and atrial fibrillation were significantly associated with SDB of stroke patient. The SDB patients showed worse stroke outcome than those without SDB. Particular lesion location or specific stroke syndrome did not show any correlation. Higher NIHSS and bulbar involvement in SDB group seem to show the influence of ischemic stroke on the increased SDB incidence.
